Who Is at Risk for Type 2 Diabetes?

Type 2 diabetes affects more than 37 million Americans, according to the CDC, and nearly 1 in 5 doesn’t know they have it. The risk factors are well established: being overweight or obese, having a family history of diabetes, being physically active less than three times a week, having high blood pressure, and being over age 45. Race and ethnicity also play a role—African American, Hispanic, Native American, and Asian American populations face higher risk. Women who had gestational diabetes or gave birth to a baby weighing more than 9 pounds are also at increased risk.

One risk factor that often surprises people is sleep. Chronic sleep deprivation and untreated sleep apnea can impair glucose tolerance and increase insulin resistance. If you snore loudly and wake up tired, a sleep study may be as important as a blood test.

Age is non-negotiable, but it’s not a reason to give up. The Diabetes Prevention Program, a landmark study, showed that modest weight loss—5 to 7 percent of body weight—and 150 minutes of weekly exercise cut the risk of developing type 2 diabetes by 58 percent in high-risk adults.

Diet Strategies That Actually Lower Blood Sugar

Controlling diabetes with diet doesn’t mean never eating another potato. It means understanding how different foods affect your blood glucose and making choices that keep levels steady. The glycemic index (GI) is a useful tool: foods with a low GI (55 or less) raise blood sugar slowly. Potatoes, especially baked or mashed, have a high GI—about 85 for a baked russet. But boiling potatoes and cooling them before eating lowers their GI because resistant starch forms. Pairing potatoes with protein or fat—like a baked potato with Greek yogurt instead of butter—further blunts the spike.

For people with diabetes, potatoes aren’t forbidden, but portion size and preparation matter. A 2019 study in Diabetes Care found that replacing mashed potatoes with a non-starchy vegetable like spinach or broccoli improved blood sugar control in people with type 2 diabetes. The takeaway: fill half your plate with non-starchy vegetables, a quarter with lean protein, and a quarter with carbohydrate foods—including potatoes when you want them.

The DASH Diet for Diabetes

The DASH diet (Dietary Approaches to Stop Hypertension) was designed for blood pressure, but it’s also a strong choice for diabetes. It emphasizes fruits, vegetables, whole grains, lean protein, and low-fat dairy while limiting sodium, sweets, and red meat. Research shows DASH can improve insulin sensitivity and lower A1C. A typical day on DASH includes 6–8 servings of grains, 4–5 servings of vegetables, 4–5 servings of fruit, and 2–3 servings of low-fat dairy. It’s not a deprivation diet; it’s a pattern.

What about the diabetes-specific plate method? It’s simpler than counting carbs and works well for many people. Use a 9-inch plate: half non-starchy vegetables, a quarter protein, a quarter starch. Add a small serving of fruit or dairy on the side. This approach naturally controls portions and carbohydrate intake without obsessive tracking.

Medications and Support: What to Know

When diet and exercise aren’t enough, medication becomes part of the plan. Metformin remains the first-line therapy for most people with type 2 diabetes, and it’s inexpensive—often less than $10 a month at pharmacies like Walmart or via generic programs. Newer drugs like GLP-1 receptor agonists (semaglutide, sold as Ozempic) have gained attention for weight loss and cardiovascular benefits. But does insurance cover Ozempic? It depends on your plan. Medicare Part D covers semaglutide for diabetes but not for weight loss alone. Commercial plans vary widely; some require prior authorization and a documented A1C above a certain threshold. If coverage is denied, ask your doctor about patient assistance programs or manufacturer coupons.

For those who need insulin, home delivery services and subscription models have made access easier, though costs can still be high. The American Diabetes Association’s Insulin Affordability program provides resources, and many states have capped insulin copays at $35 per month for insured patients.

Diabetes neuropathy treatment is another concern. About half of people with diabetes develop some form of nerve damage, typically in the feet. Medications like gabapentin or duloxetine can help, but blood sugar control remains the most effective way to prevent and slow neuropathy. Annual foot exams are essential.

Small Steps, Big Impact

Preventing or managing type 2 diabetes isn’t about perfection. It’s about consistency. A 10-minute walk after meals can lower post-meal blood sugar by up to 30 percent. Swapping sugary drinks for water or unsweetened tea can cut hundreds of calories a day. Reading labels for added sugars—aim for less than 25 grams per day for women and 36 grams for men—makes a difference.

If you’re at risk, ask your doctor for a fasting blood glucose test or an A1C test. The CDC’s National Diabetes Prevention Program offers lifestyle change programs across the country, often free or low-cost. These programs provide coaching, group support, and practical strategies for eating better and moving more. They work: participants typically lose 5–7 percent of body weight and significantly reduce their risk of progression to diabetes.
